Supreme Court Rule 34.1 lists the elements that must be included on the cover page of the brief, including the docket number of the case, the name of the court in which the case will be heard, the caption of the case, the court of origin of the case, the type of document, and the name of the counsel of record.
Similarly, Rule 341(d) of the Illinois Civil Appeals Rules lists the elements that must appear on the cover page of a brief, including the case number, the name of the court that is being appealed to and of the tribunal whose decision is being reviewed, the name of the case as it appeared in the lower tribunal, with the parties’ identities indicating their status in both courts (e.g., “plaintiff-appellant”), the name of the trial judge entering the judgment to be reviewed, and the individual names and addresses of the attorneys and their law firm.
