General Recommendations and Conclusions

If better control of paratuberculosis through de­tection of infected herds is desired, liquid culture methods, which are much more sensitive than solid medium culture, will need to be used more widely.

Commercial BACTEC 12B was the only liquid medium shown to support the growth of a wide variety of MAP strains. Since BACTEC 12B was discontinued, a fully validated alterna­tive has been made available to the international community. International standards for culture protocols are required, as there are many vari­ations of protocols, which have significant ef­fects on sensitivity and prevent the meaningful analysis of new diagnostics for which culture serves as the gold standard. Contamination of faecal cultures remains a major hurdle to timely and economical culture of MAP and improved decontamination protocols would be a major advance, enabling greater use of liquid culture. The ongoing widespread use of solid media for diagnostic culture masks the true prevalence of MAP as well as strain diversity, and results in exaggerated estimates of sensitivity for other diagnostic tests that are compared with culture. Failure to routinely employ liquid culture meth­ods to more fully explore the role of MAP infec­tion in Crohn's disease is inexplicable.
